What do you do when every door you were supposed to walk through slams in your face, and the only one left open is on a chaotic little island at the edge of the map?

This is the truest story I've ever told, and the credits give it away: they're real photographs of real students, on a real island. The film opens with a title card, "Most of what you are about to see really happened,” and I mean it more here than anywhere.

The Caribbean, 1982. Dean and his best friend Bruce, rejected from every American medical school, arrive at a ramshackle offshore med school that is nobody's dream and their only remaining option. A ruthless dean gives Dean three months to prove he belongs, or he's out, with no recommendation and no future in medicine. What follows is heat and sugar cane and chaos and friendship, a couple of young men with everything to prove and nothing to fall back on, learning medicine in a place that looks nothing like the sterile institutions that turned them away.

Underneath the comedy is my own father, here, as in so much of my work, an immigrant who built himself out of nothing and expected his son to do the same, whose approval was the thing I was really chasing across an ocean. My father is always riding me, Dean says, like nothing is ever good enough. I know that feeling. I also know where it leads, if you're lucky: to the far side of three months, and four years, and a diploma waved in the air.

It's the origin story of a surgeon, the beginning of a path that runs through several of my films. But mostly I wrote it as a love letter: to the friends who went through the fire beside me, to the unglamorous back door that turned out to be the right door, and to everyone who was told they'd never make it, and quietly, stubbornly, did.
